The authors present 2 cases with unusual findings of aberrant regeneration of the third cranial nerve palsy with coexisting sixth cranial nerve palsy. The first case was a 44-year-old male with head trauma from a road traffic accident. The second case was 25-year-old male also with head trauma from a road traffic accident. Both had synergistic adduction during attempted elevation. Surgery involved medial rectus recession and single muscle partial transposition to address the hypo eye adducting in elevation. Postoperatively, patients were ortho in primary gaze with no diplopia and with reduced synergistic adduction.
