This study presents results of bilateral superior plication of the lateral rectus for treatment of adult onset esotropia, as a less invasive surgical option. This was a retrospective study of 15 patients with mean age of 51.5 ±12.6 years; 67% female. Mean onset for diplopia was 45.5 ±33 months prior to surgery. Pre-surgery, 87% had used prisms. Preoperative mean deviation was 25.7 ±11 prism dioptres (PD) at distance, and 19.4 ±16PD at near. Mean follow-up period was 9.10 ±6.1 months. Postoperative mean deviation was 0.16 ±0.80PD at distance and 0 ±0PD at near; significant at p=0.001. There was just 1 postoperative esotropia measuring 6PD. The remainder were ortho measurements. All had no diplopia postoperatively and no prisms were needed. There were minimal long-term complications. Limitations of this study were its small sample size, no control group and variable follow-up periods. Future, larger studies are required but this shows potential as a promising surgical option.
