The aim was to examine the risk of retinal detachment in children with myopia over a 10-year follow-up period. This was a retrospective cohort study using the TriNet X platform which houses anonymised electronic patient records from 180 healthcare groups. Two cohorts were established: (1) children with myopia (n=151,499) and (2) a control group of 5,073,124 children without myopia. At baseline (pre-propensity score), myopes were older than controls (10.9 vs 5.8 years). Myopes had higher numbers of black and African American children (26.6% vs 18%). Children were then matched with a 1:1 propensity score to balance age, gender and race. Each cohort had 145,814 children. The primary outcome was incidence of retinal detachment. Retinal detachment was noted in 0.5% in myopes and 0.04% in non-myopes. The risk difference was 0.004 with a risk ratio of 10.43 and odds ratio of 10.47. Findings support the need to actively manage childhood myopia to reduce the longer-term risks of myopia.
