This prospective study evaluated superior rectus recession with Y-splitting for treatment of dissociated vertical deviation (DVD) looking at aspects of efficacy and complications. The study included 12 patients; 9 males and 3 females with mean age of 14.5 ±13.31 years (range 3.5 to 48 years). There were no inferior oblique over actions. The surgical procedure was undertaken on 19 eyes. Mean postoperative follow-up period was 6.9 months. Overall, the success rate was 100% (defined as within 10PD of ortho); 2 achieved after reoperation. Adverse events included intermittent and initially noticeable hypotropia of the operated eye in 1 patient with persistent -3 limited elevation in abduction and adduction; mild limited superior rectus action in another 2 patients; appearance of DVD in the fellow eye after unilateral surgery in 2 patients. The authors therefore recommend bilateral surgery for DVD and conclude that superior rectus recession with Y-splitting is a promising surgical option for treatment of DVD. This warrants further research.
