In this study, the authors aimed to categorise the demographics and clinical characteristics and medical evaluation results in patients with adult acute onset comitant esotropia (AACE). This was a retrospective study of 268 patients; 62.7% female, 84% white, with mean onset at age 53.7 ±19.3 years. A bimodal distribution was noted with peaks in the second and sixth decades. Diplopia was present in 95.5% and 30% had had cataract surgery with mean refraction in the phakic group of -3.22 ±3.84D (68% myopes, 16.5% hypermetropia and 9.5% emmetropia). Mean distance esotropia was 17.2 ±13.1PD at presentation and 19% were divergence insufficiency type. Highest angle measurements were in the 18–29-year group versus the smallest in the eighth decade. Most were divergence insufficiency types in late ages. Medical work-up was usually non-revealing. However, cases were found with thyroid eye disease, myasthenia gravis and neurological disease, mainly in women.
